If you're near someone who is talking to himself/herself, do you have to make a comment or can you just consider them crazy and ignore them?

There's "getting older talking to yourself" and "scary crazy talking to yourself". Which one was this?
this was, "I am looking for some attention and I hope you hear me and talk with me so i can feel special"
Doh! Forgot about that one.
However, they usually fall under the "scary crazy" eventually. Baby steps!
