Yeah I think the overall American answer is grilled cheese. Although with ham in it some would argue it's a "melt"
I don't remember the details but I would absolutely believe I might have been the one who opposed to calling it a toast because it sounds like something I'd do LMAO

Yeah I think they got blended together as terms a bit when companies started making one you could swap out which pattern it uses.
