There's not a lot of details cause it's so much of an art than science. But think of it as being able to read the pulse of a person and telling you how their state of body is.

Chinese medicine is very confusing even in real life. Imaginary Chinese medicine with qi is even more confusing.
Ok so how about he can sense that it was disrupted a long time ago, like its original path around the body was messed up and it had to make new paths so it feels kind of wonky but also like... that’s its path now. The old paths are like scars. Something to that effect. So he can sense that they’re not “right” per se, but he can’t do anything
about that either.

I would compare it to HP and MP for a character in a game, except you'll also die if you run out of MP because that is the life force. And it has a whole system of "meridians" that's the same as blood vessels that travel throughout the body.

So compare what a person could read out of doing an MRI of the body's blood vessels, but here he's doing it by mental version of investigative laparoscopy.

So he'll see blockages, narrowing areas, damaged areas, things that are interfering with the flow of qi to areas of the body and interfering with a healthy body from being healthy.

Assuming we go with the idea that qi and meridians are the same.
That works!
So the damage wouldn’t block qi completely anywhere but would have noticeable damage in the abdominal cavity and the brain. Abdominal not just digestive system but liver, spleen etc.

The question is what sort of damage do you want him to see? And do you want them to be able to work on a way to fix it? He won't be able to instafix anything, but they can work on improving things. Very arbitrary what you want Malcolm to get better in if any.

Brain he won't be able to do much for. But body he can help with in mild ways. Or teach Malcolm how to regulate qi and help himself as well in some ways.
Maybe more of a narrowing than blockages. Like some of the meridians are a bit shrunk or shrivelled?

Wu Xin can be surprisingly dependable sometimes
His current panic attack self-soothing which he has fallen back on since childhood is squeezing his eyes shut and breathing deeply a few times until he can’t hear his heart inside his head anymore 🤣
