They changed their setup a few years ago and no longer charge a setup fee. They take a tiny percentage of each sale. You do have to buy your own ISBN through Bowker; no freebies from Ingram Spark. They've also initiated the Share and Sell links, where the general public can order books directly from them instead of a retailer.
Oh, in addition to ditching Google Books, I'm not offering returns on print copies for this one. I intend to go back and stop allowing returns on the first 7 books at some point, too. I'm tired of a small number of returns/canceled orders eating most of my royalties every year.
