You need that when the sites are large and complex. I usually deal with a lot start ups/record labels. My style tends to attract those.

in fact, if sites aren't big it is even more important, they generally don't have a huge budget for PPC..

It is amazing how the internet world has evolved. I still remember when you could do simple HTML and get paid a very lucrative amount.
