The brackets indicate action usually, but Nill is psychic so in her case it's usually Yosuke's inner dialogue that she's reading.

Basically it would be the prose if we were doing it prose style and the unmarked text is dialogue. Nill's is in italics because she thinks it instead of speaking it

Nope, neither of them are actual ghosts. Yosuke is fading away because of a game mechanism. There's a weird sort of gravitational force that brings people to the island where the game takes place.
